This fill-in-the-blank form provides a good overview of the basic legal questions that you should have in your parenting plan, clearly spelled out.

The intent is for Pro Se and uncontested divorce filings, but as a template to make sure you have all of the bases covered in your own custom plan, it's a decent checklist to make sure you have equivalent clauses in your own plan.
